The Kraft Prize for Excellence and Innovation in Community Health, hosted by Mass General Brigham, honors transformative organizations, programs, or innovations that significantly impact community health outcomes and have the potential to scale. The prize more specifically recognizes efforts focused on addressing cancer, cardiometabolic disease, maternal health, and substance use disorder. It also recognizes innovations improving healthcare access, quality, and social determinants of health. 

Eligible applicants must fall into the categories of: community health practitioners, healthcare professionals, non-profits, or private-sector innovators based in the U.S. One winner will receive $100,000, individualized mentorship and a presentation opportunity at the Mass General Brigham World Medical Innovation Forum. Two finalists will each receive $10,000 and an invitation to the forum.
